I wish to Gift Aid my donation and increase its worth by 25% at no cost to me I am a UK taxpayer and would like The Myton Hospices to treat this donation, any donations I make in the future, and any I have made in the past four years, as Gift Aid donations until I notify you otherwise. I understand that if I pay less Income Tax and/or Capital Gains Tax than the amount of Gift Aid claimed on all my donations in that tax year, it is my responsibility to pay any difference.
